Try resting after couple of games. Right after you winning streak you've started feeding slightly more than you normally would, so I thought you kept playing even while being dead tired.
Pick different heroes, which likely will have more impact on the game. Sometimes phoenix doesn't work as good as it usually does. Pick something reasonable - suiting your lineup and ruining opponent's.
Die less. Like, as less as possible. Though don't let you core die.
You don't have plus, so I have no idea about warding and staff, so I will just skip it, but don't underestimate the vision profits.
Utilize the map better. Your gpm/xpm could have been better.
Again - don't feed, and you'll be okay.

Hi guys. These days when I am playing ranked I noticed that I go on a winning streak followed by a losing streak. I don't have consistency at all. Would you guys be kind enough to help me out by pointing out a few specifics as to what I can do.
